1. Certified True Copy of the birth certificate or certified transcription or the certified true copy of the page of the registry book containing the entry/ies sought to be corrected;
Note: Both the Philippine Statistics Authority (PSA) copy and Local Civil Registrar's Office (LCRO) copy shall be submitted to determine the consistency of entries in both copies.
2. Earliest school record or earliest school documents. In case where the petitioner/document owner never entered school, an affidavit attesting to the facts shall be submitted;
3. Medical records; in case where the petitioner/document owner has no medical records, an affidavit attesting to the facts shall be submitted;
4. Baptismal certificate and other documents issued by religious authorities. In case where the petitioner/document owner has no baptismal certificate or similar documents, an affidavit attesting to the facts shall be submitted;
5. Clearance or a certification that the owner of the document has no pending administrative, civil or criminal case, or no criminal record, which shall be obtained from the following: Employer (if employed), NBI and Philippine National Police;
Note: For minor (under 18 years old), such clearances shall be complied with only if required by the City or Municipal Civil Registrar.
6. Notice or Certificate of Posting (in case of migrant petition, the petition shall be posted first at the receiving City or Municipal Civil Registrar for ten (10) consecutive days before sending it the proper City or Municipal Civil Registrar, and upon receipt the proper City or Municipal Civil Registrar will post again the petition for another ten (10) consecutive days;
7. Proof of Publication (copy of the newspaper clipping of the published petition and affidavit of publication from the publisher);
8. Medical Certification issued by an accredited government physician that the petitioner has not undergone sex change or sex transplant, In addition, the City or Municipal Civil Registrar shall issue a certification of authenticity of the medical certification issued by the accredited government physician;
9. In case a person applying for correction of sex is born abroad, the applicant/document owner shall present a medical certification issued by an accredited government physician in the country where he/she was born. The Consul General shall certify the medical certification and shall issued a certification of authenticity of the said medical certification.
