JOINT AFFIDAVIT OF TWO DISINTERESTED
PERSONS

after having been sworn to in accordance with law, hereby depose and state:

first name;

in Birth Certificate), as appearing in her birth certificate, and (Name Known or in Official Documents),
as appearing in some of her official documents, including her school records
and marriage certificate, refer to one and the same person;
4. We
are not related by consanguinity or affinity to her;
5. We
are executing this affidavit to attest to the truth and veracity of the
foregoing facts and for whatever legal and beneficial purposes this may serve.
