2. There is no entry under Middle Name;
3. On the other hand, in my official records including school documents, SSS papers, and work records, my name is indicated as (NAME OF AFFIANT AS WRITTEN ON HER OFFICIAL SCHOOL AND WORK RECORDS), with a Middle Initial;
4. Both names/documents pertain to one and the same person;
5. The difference stems from inadvertence or because of my mistaken notion that I carry a middle name;
6. I am executing this affidavit to attest to the truth of the above statement and for whatever lawful purpose it may serve.
