JOINT AFFIDAVIT OF PARENTAL CONSENT
We, Spouses

married to each other, Filipino, and residents of (Place of Residence), Philippines, after having been duly sworn in
accordance with law, hereby depose and state:

That we are the parents of (NAME OF CHILD), a minor at 12 years of
age, Filipino, living with us and under our parental care and authority, and
who is going for a trip abroad with her grandmother, (NAME OF RELATIVE WHO WILL ACCOMPANY THE CHILD), specifically to Dubai,
United Arab Emirates, to attend the birthday celebration of the undersigned’s brother
and his wife and the baptismal of the couple’s baby girl;
2.
That we cannot personally travel with our
daughter due to work commitment;
3.
That we are giving our full consent to the
travel of our daughter to Dubai, UAE to attend said important family occasions;
4.
That our daughter will be accompanied in
her travel abroad by her grandmother, (NAME
OF RELATIVE), Filipino, a resident of Cebu, Philippines, who possess the
financial capacity to finance the expenses to be incurred during their travel and
with whom she will stay while in Dubai, UAE;
5.
That we are hereby giving our full consent
to our mother (NAME OF RELATIVE) to
accompany our daughter in her travel abroad, and together with our mother we
will shoulder all the expenses to be incurred during the travel of our daughter;
6.
That we are executing this affidavit to
declare the truth of the foregoing facts and for whatever legal purpose it may
serve.
