JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS
WE, (Name of Affiant 1) and (Name of Affiant 2), both of legal age, Filipino, and with respective residences at (Residence of Affiant 1) and (Residence of Affiant 2), after having been sworn to in accordance with law, hereby depose and state: THAT –
1. That we personally know Jamaica Anne Dimailhan, who is our personal acquaintance, and we are fully acquainted with her circumstances, activities, and reputation;
2. That Jamaica Anne Dimailhan, as appearing in her official records, identification cards, and other pertinent documents, is one and the same person as Jamaica Dimailhan;
3. That Jamaica Anne Dimailhan is commonly known, recognized, and called “Jamaica Dimailhan” or simply “Jamaica” or "Jam" in informal settings, day-to-day interactions, and in the community;
4. That the omission of “Anne” in some instances when referring to Jamaica Anne Dimailhan is not intended to, nor does it in any way, establish a separate identity but is merely a shorter, informal version of her full name;
5. That we execute this joint affidavit to attest to the truth of the foregoing facts and for the purpose of confirming the identity of Jamaica Anne Dimailhan as one and the same person as Jamaica Dimailhan, and to clarify any confusion that may arise due to the differences in name usage in different settings and documents;
6. We are not related by consanguinity or affinity to her;
7. We are executing this affidavit to attest to the truth and veracity of the foregoing facts and for whatever legal and beneficial purposes this may serve.
