AFFIDAVIT OF NON-PATERNITY


I, CHOY T. PAKLONG, Filipino, of legal age, married, and a resident of Bondai, Talisay City, Cebu, after having been sworn to in accordance with law, hereby depose and state:

1.    I am the legal husband of Chimay T. Pipay at the time of the birth of the latter's child: ____________________________, who  was born in _________________ on ____________________.

2.    I acknowledge that I am not the biological father of the above-named child;

3.    Although a presumed father, I acknowledge that I am not the biological father of the above-named child. I understand that by signing this affidavit I may be giving up certain rights if the presumed father is not the named the father of the above-named child;

4.    I do not want my name to appear on the child's birth certificate and other official documents;

5.    I have not provided the birth mother with child support or pre-birth support; I have not provided her with prenatal care or assisted her with medical expenses; I have not provided the birth mother or her child or unborn child with support of any kind, nor do I intend to do so;

6.    I do not object to this affidavit being filed with the court, any government agency or foreign consulate even if I am not present. I understand and agree that the agency or institution concerned, based on this affidavit, may enter a judgment or resolution declaring that another man is the child's father.

7.    The information above is true of my own personal knowledge and belief.
