the recipient of a Fedex Express package from Seoul, South Korea to Cebu,
Philippines, with Tracking Number 012345678 and Master Airway Bill (MAWB)
No. 87654321, ETA or estimated time of arrival on November 11, 2015;
2.
The
contents of said package or parcel are exactly as described in the Airway Bill,
to wit: NEBULIZER + AIR FILTER CARTRIDGE - FOR AIR PURIFIER with total
value for Customs at US$150.00, a copy of said Airway Bill and Commercial
Invoice are attached herewith;
3.
The
said items are sent as part of my personal belongings in connection with my business in the
Philippines and for my prescription medical care and personal needs;
4.
To my
knowledge, it does not contain any illegal items or contraband under Philippine
law;
5.
I
herein attest to the truth of the foregoing facts.
